The following forms are available for download in Adobe Acrobat format. All forms can be viewed, typed into directly, and printed using the Adobe Acrobat Reader, which is free from Adobe. If you don't already have this feature, download a free copy of Adobe Acrobat and install it on your computer.

These forms cannot be sent to the Foundation electronically, because we require original signatures on each form, and in some cases receipts must be attached. Please complete all information on forms to avoid delays, as incomplete forms will be returned to the originator. Forward all forms to the PSU Foundation via campus mail (code FOUND) or via U.S. Mail at P.O. Box 243, Portland, OR 97207-0243.

Account Agreement
This form is used to request the establishment of a new account. Because of the cost of maintaining Foundation accounts, we request that you not open a new account unless you anticipate a minimum initial deposit of $500. If you are interested in opening an endowed account, this form must accompany a completed Endowment Agreement form. Original signatures are needed on this form.

Account Signature Card
This form is used to update authorized signers on existing accounts. Newly submitted forms will replace existing records, so please make sure all authorized signers are listed on the update.

Check Request
This form provides the necessary information to request payment to a vendor. This form can also be used to reimburse a PSU Index account from Foundation account funds. Original signatures are needed on this form.

Confidentiality Policy Form
This form must be completed and received by the PSU Foundation before access to granted to Summit software or Crystal Enterprise web reports.

Contact Report
This form is used to report contacts made with donors, and friends of Portland State University.

Electronic Funds Transfer Authorization Agreement
This form provides a convenient way for individuals to make a pledge to any Foundation account or the general unrestricted fund, and have the pledge payments deducted directly from the donor's bank account through Electronic Funds Transfer (EFT).

Endowment Agreement
This form is for informational purposes only. All endowment agreements must originate with the Foundation. Please contact Becky Hein, heinbe@pdx.edu or x5-5881 for assistance.
This form is used to request the establishment of a new endowed account for a purpose other than scholarships, such as a Lectureship, Professorship, or support of a department or program. This form must be accompanied by an Account Agreement form. Endowment accounts are created by donors who require that the fund is of permanent duration with an annual appropriation for expenditure to support a University program or activity. Only the donor can stipulate that their contribution is to be endowed. Original signature is needed on this form.

Endowment Agreement with Scholarship Addendum
This form is for informational purposes only. All endowment agreements must originate with the Foundation. Please contact Becky Hein, heinbe@pdx.edu or x5-5881 for assistance.

Faculty/Staff Expense Report & Reimbursement Request
PSU faculty and staff members wishing to be reimbursed for expenses, should complete this form and forward to the Foundation. Original signatures and receipts are needed on this form.

Funds Transfer Request
This form provides the necessary information to transfer funds between PSU Foundation accounts.

Gift Transmittal Form
This form provides the necessary information to accurately make a deposit into a Foundation account.

Information Request Form
All Information Requests must now be made through the Report Request link in Apex Reports. Please contact Teri Dukes Wingard, wingardt@pdx.edu or x5-4917, if you need access to the Apex Reports.

In-Kind Contribution Report
Information obtained from this form insures an accurate record of non-cash contributions, such as equipment, books, artwork, or services, made to the Foundation. Please complete and forward to the PSU Foundation within 7 days of contribution. A thank you letter will be generated from this form.

Payroll Deduction Form
This form provides a convenient way for PSU faculty and staff members to make a pledge to any Foundation account or the general unrestricted fund, and have the pledge payments deducted directly from their paycheck.

Personal/Professional Services Agreement
This contract allows University departments to hire non-PSU employees to complete services not available from existing University resources. Contractor cannot be employed by PSU at the time contract is in effect. Original signatures are needed on this form.

Pledge Agreement Form
This form provides donors with a simple way to make a donation pledge to the PSU Foundation. Original donor signatures are needed on this form.

Proposal Internal Approval Form (PIAF)
This link will take you to the Office of Research and Sponsored Projects Forms site. The Proposal Internal Approval Form is the primary means for gaining institutional approval for a research proposal. It is a 2-page form to be filled in with general proposal details such as PI and sponsor contact information, budget, and project dates. It must be signed by the Principal Investigator, Department Chair, and Dean. Once these three signatures have been obtained, the PIAF can be sent to ORSP for final approval and signature. This form must accompany all grants applied for in the PSU Foundation's name.

Public Funds
Complete this form when you receive public funds (Federal, State, County or City), and submit within 7 days. Please indicate full amount of funding, even if it will not be received all at once. Use only for funds to be applied to campaign priorities.

Report of Gifts or Grants to PSU
Information obtained from this form pertaining to gifts deposited into PSU Index gift accounts will be added to the fundraising database. This form is for informational purposes only, and allows University faculty and staff to access complete donor activity records which includes all gifts made to the University and the Foundation.

Scholarship Award Notification
This form provides the necessary information for the processing of scholarship awards. Please submit two copies of form to the Foundation, and keep one copy for your records. The Foundation will forward Award Notification to the Office of Financial Aid after verifying fund balance. Original departmental signatures are needed on this form.

Stock Transfer Form
Completion of this form allows for electronic transfer of stock gifts to the PSU Foundation. Note: Please send one copy to the Foundation, send one to your broker for completion, and keep one for your records. Original donor signature is needed on this form.

Web Access Request/Change Form
